Root Cause Analysis
It is common that Root Cause Analyses (RCA's) consists of story-telling and/or conjecture, focusing only on surface-level symptoms. In these cases, solutions as "Re-Train" or "Re-communicate" become common, albeit ineffective. This presentation will outline the essential elements for a Root Cause Analysis (RCA) that will expose and eliminate deeper seeded causes by generating creative and cost-effective solutions. Through a structured approach to RCA, the cause and effect relationships are exposed unlocking a wide array of solution options including both corrective and preventive.
Chris Eckert is the co-founder and President of Sologic, LLC, a global root cause analysis training, software and incident investigation company. Since 2011, he has managed the day to day operations of Sologic while also working directly with clients to deploy enterprise-wide root cause analysis programs and within the supply chain.
Chris has significant experience in establishing root cause analysis (RCA) programs in a wide variety of industries and has facilitated hundreds of investigations ranging from minor injury to catastrophic failure. Over the past 30 years, Chris has authored numerous papers and has delivered many presentations at international industry/trade association conferences on the topics of incident investigation and root cause analysis. He has been a certified RCA instructor since 1999.
Prior to Sologic, Chris was President of Apollo Associated Services from 2002 to 2011. Chris also has 10 years of experience with Dow Chemical where he held positions in manufacturing, design and reliability engineering. Chris also spent 4 years with Rohm and Haas in their corporate reliability group where he was responsible for implementation of reliability improvement programs at site and enterprise levels across Rohm and Haas' global operations.
